Calibre 0.9.20

In paged mode Calibre ignores the top margin of the first block of body text.

This means that book titles, chapter headings etc are located at the very top of the page -- spoiling the layout.

Unticking the Override Page Margins option doesn't seem to do much apart from greying out the margin settings.
