From the menu bar, under Tools Metadata Editor--

I entered the title of my book here, when I started my book, and it has been used in all new xhtml documents. That's fine.

But then I changed the title, and now all new xhtml documents continue to use the old title.

Am I doing something wrong, or should I report this as an Issue.
