Just update to version 0.9.23
Click on "Preferences" --> Click on "Toolbar" (in Interface section) --> Click to choose toolbar or menu to cutomize --> click on "The main toolbar" --> Move "Edit ToC" from "Available Actions" column on left to "Current Actions" column on right --> OPTIONAL you can use the arrow keys to adjust where it appears on your toolbar --> Click on the green "Apply" button and you're all set.
This sounds much more complicated than it actually is. It's pretty straightforward.
