I think this maybe has something to do with Adobe Digital Additions.
I copied and pasted the book from where the the Adobe put it into my Calibre-Add folder and nothing happened with the auto add feature, I then tried to manually add, still did not work, so then I tried to manually add it from the original folder where Adobe put it, and no problem.
When I tried to add it from where I copied the book to, Calibre did not even "see" the book in the Calibre-Add folder.
