Thanks for the offer - so I have a proper audit trail I'll rebuild the book from scratch and then transfer it as kepub, recover it from the Glo and then post a copy here (original from Gutenberg so shouldn't be any legal issues).
That way all the intermediate tweaking I've done doesn't mess up what seems to be the problem.
If you also want the "non-kepub" version let me know.
