As Peter said, there is a bug in the driver where some books don't match. It generally means the title of the book on the device doesn't exactly match what calibre has. This usually comes about because of a plugboard adding the series info or because an article is moved to the end of the title.

I'm working on it and might have just checked in something to fix it. Or, more likely reduce the times it happens.

When you put a book onto a Kobo device, it gets processed and added to the internal database and hence the library. When you replace that book, it doesn't get reprocessed in the same way. So, changing the metadata such as the comments won't get picked up. And using a plugboard to update the title with the series info won't get picked up. But, putting them on the shelves is handled by the driver. This will get updated when calibre does a sync.
