If you mean changing the cover within the metadata editor, that just places the image file into the directory that epub is in in your calibre library, it doesn't change the epub itself, so it doesn't get copied to your Touch (or any other device). To actually change the cover inside the epub, you have to either do an epub-to-epub conversion or get yourself 7Zip, or some other archive handling program, open the epub like you would a .zip or .rar, then replace the existing cover.jpg in the base directory, if there is one (if there isn't, you'll have to do a conversion with calibre as adding a cover to an epub that doesn't have one can be a major pain to do manually).
