For the book with the text only cover showing, that sounds like an error. The text only cover that I think you are seeing will have the book title or file name plus the book type under it. Under normal circumstances, I have only seen this for text files. It might also be used for HTML and RTF, but I'm not sure.
The other time you see this text cover is as the cover images are generated. When new books are put on the device, the home screen can flash the text cover and then replace it with the generated image. You will also see this each time a book from the SD card is displayed in the home screen.
The fact that you are seeing this for a book means something went wrong. If the first page displayed OK when you are reading it, the device should be able to use it for the cover. That leaves a bug or maybe running out of space to store the image or memory. Does it still use this after connecting to USB or doing a restart? Either of those should cause a recreation of the cover image if one isn't stored.

Odd, the first thing I did when I saw the cover wasn't showing was to reboot, and it didn't work. I just rebooted again, and now the cover is showing. And yes, it was showing the title and type of file.
So if the KT doesn't use the cover images uploaded, what are the options in the driver for?
If I understand correctly, when changing the cover, Calibre inserts it as the first page? If so, if I change the cover on a book already on my KT, do I need to delete the book from my KT and add it again so it will update the cover?
