if I'm editing an RTF with Word or PDF with Acrobat, and I change the book title or author in the book list (not sure about metadata editor) then calibre does as I describe above.

But, I of all people, should understand why its daft to change those particular fields whilst the RTF and or PDF is open for editing. Maybe its only Word and Acrobat - perhaps other editors (eg Sigil) don't hold any locks in which case...
