This has just started happening to me, too, on a Kindle Touch running 5.1.2. I transfer a text file to documents, and when I disconnect the USB drive it appears on the Kindle's home screen. At the top it says My Items (10). I can open the document and read it. On returning to the home screen it's gone and it says My Items (9) at the top. The document is not in any collection, but the text file and the .sdr are still there.

After restarting the Kindle, or after connecting to USB and disconnecting, the document reappears for about two seconds, then it disappears again.

I tried adding the document to a collection during those two seconds. The collection now says "3 items" and when I open it I see "(3)" at the top, but only two items listed.
