The treatment of phobias quite frequently involves a rather sadistic technique known as flooding. In the case of fear of flying, the patients are put in a simulator, then a real airplane and spend as much time as needed to allow the anxiety response to be extinguished. It is painful, but extremely effective. Lots of informed consent documents need to be signed beforehand...
