Van Dyke, John C.: A Text-Book of the History of Painting, v.1, 20 July 2008
First published in 1894, this 1909 edition includes over 110 black-and-white illustrations. The HTML file at Project Gutenberg includes links to modern color images of the paintings: http://www.gutenberg.org/etext/18900
