You have the option - deleting the book removes both files and metadata, using 'remove files of a specific format' deletes only files and leaves empty book (no formats, just metadata) in the library. If you go spelunking in the folders without using Calibre, then you may break your library. Calibre can't keep current data about your library if it's not informed of the changes.
