I've used SplashID in the past. The iPhone version is a good first start, but the UI needs work; too many finger presses to see what you're looking for.

I've also used KeePass on a USB keychain with the Portable Apps suite.

More recently, I've been using OneNote stored on a USB drive for that data I only need when I'm at my computer.
