DSL with 'up to' 12mb (although the most I've gotten is 9), landline phone with unlimited long distance included, with taxes and such I pay $86/month. I don't have cable - I stream Netflix and Hulu for my TV, so I guess I'm a pretty heavy user, but I don't know how much I actually use. There's no caps or throttling.

I don't have a smartphone, just a cheapo cell from TracFone that lives in the car for emergencies and costs $5.99/month.

I'm amazed at the cheap Internet most of you Europeans have, since everything else over there seems so much more expensive. Do your governments subsidize it?
