I have just updated the initial post, with a link to v. 1.06, which includes:
1) solved a CDATA bug in metadata;
2) added --spine and --toc options;
3) better examples in the zip file.
The GUI (and Calibre plugin) must wait until the next release, since I am quite busy job hunting...
