Can anyone help me with this issue. When I convert a book from html to epub; Chapter 1 is always skipped in the table of contents.

All chapters are inside of the <h2> tag and proceeded by <mbp:pagebreak />.

Example:
<mbp:pagebreak />
<h2 align="center">CHAPTER 1</h2>
