Where it falls down specifically is when text is simply missing from the ebook; this is extremely common. Eg, when I created my edition of Dickens' "The Old Curiosity Shop" from the PG source I found two places where a complete double page of text had been omitted, as well as several missing paragraphs and innumerable missing words.
