Well, one option is to have the author sign a body part with a sharpie, then go to a tattoo studio you trust and have the artist tattoo it in.

BookieJar has a way to produce signed ebooks as well, but I've never bothered to look into it. I'd rather sign a pbook, even if it is just a piece from Lulu.com or Createspace.
