The nook keeps a cache of thumbnails for fast display. When you delete the book from the nook (if you do it via the PC), make sure you unmount the nook and let it refresh the library.
I have not tried changing covers, but I have updated books that did not have a cover (which I added via Sigil, similar to what you do), and following these steps it worked for me.
