My sister was being harassed on her home phone by a "collection" agency that claimed to be supported by a law firm. She spoke with them and set up a payment plan. Before sending any money she spoke with me and I said that was great, but get the law firm to send the agreement in writing. Instead, she got some generic letter from a law firm in Las Vegas. I was curious and Google Maps came to the rescue. It was an address in a strip center in Las Vegas and when you went to "street view" there was no sign of the law firm. So I called the business next door to the address that was given for the law firm and they said that they had no idea who those people were, but that the address referred to a closed dry cleaner from months before.

The scary thing is that I know, and I am pretty sure that you have knowledge of how easy it is to use software like Asterisk to run your phone center and that right there in the software is a setting for what you want to show on the caller ID. She still gets calls from them, even after explaining to them that she knew what they were doing. She occasionally blocks the number and they spin a new one within a few weeks. She doesn't really care. Just an interesting note on the Las Vegas thing.
