Wrong Cover Image

I just discovered a book's cover in the Apple iBookstore is wrong. Instead of the expected cover, my company logo appears. I use the company logo in the book on the copyright page, below the company name. I do not know how long this has been happening. Initially, the cover image was correct in the iBookstore.

I want to fix this issue. I was going to ask advice in the Apple Support forums, but I knew I would get better advice here. I will follow the proper steps in iTunes Producer v2.6 to upload a new version of the book. What do you think caused this? Bad epub structure on my part (I use Sigil)? A glitch at Apple's end? Has this happened to you?
