The only thing that works more or less successfully, at least for me, is abbyy's pdf transformer (or more or less worked since I've given up on pdfs atm).
But - and it's a big but - if pdf is complicated you have select fields manually and make pictures of formulas. Depending on the number of pages that can be rather tiresome...
