I did confirm that the card had not been removed from either device before the incident. I was just suspicious that this has only happened on the new devices that have the new firmware. I don't want to upgrade the firmware on the others if there was a "known" issue of this happening with 4.2f17. But it sounds like it isn't related so I'll have to put it down to luck of the draw.
Although I don't like it I do understand and accept that bit's of data can be lost (and that digital devices generally get upset when this happens) but it should be a rare occurance.
