In my case, the problem was that I had added books via drag and drop to the Calibre library. On Mac at least, this creates an alias (similar to Unix symbolic link). Thinking I had copied the files, I moved or deleted the originals leaving Calibre with no way to fetch the files. Correcting this, I am now able to access these files via Safari on either MacOS X or on iOS. So, pilot error.
However, neither iteration of Safari fully understands the Feed (OPDS) option. On iOS, using the address feed://yourserver.com/opds gets redirected to reader.mac.com/mobile/v1/null/opds/navcatalog/4f6e98787678 (last segment is made-up but similar to actuality) and the error message, "Server not responding." On the MacOS X desktop, Safari gets the listing By Newese and so on but there is no way to retrieve the file. Firefox and Chrome are as bad or worse.
Stanza on iOS still works for me against OPDS servers as does eBook Search and other iOS apps that are OPDS savvy. Ibis Reader, an HTML 5 web app, works fine against OPDS servers, including Calibre.
