I did one starting with a PDF last night that made a TOC. This morning I'm trying a different one but get no TOC.
If there are too many chapters does Calibre just do nothing at all?

You will get a better answer in the
Calibre forum but in order to get a working TOC you have to match the XPath correctly on your ebook. I suggest heading over there and reading up on the threads to get a better idea.
