Not always, the new Star Trek movie is an example of a new timeline created (oops, almost gave something away - go watch it).

An early time travel book was "Lest Darkness Fall" by L. Sprague DeCamp where the protagonist deliberately tries to change the past and (again, go read it).

Now in Star Trek Enterprise...
