Actually, those two options I've mentioned displayed very well in a Cybook. Other combinations would display glitches, like the one you mentioned (a blank page for every picture one).
Manga2Ereader has a GUI and is fairly easy and straight forward to use. One good thing is that a proper sized PDF is generated. And if you convert that PDF file to PRC format with the Mobipocket Creator, it exctract the resized pictures and compiles them in a single file.
