I am not Meme, but I hope you will still accept it...
Most user conjure up a cover containing the title and so on and a picture in a graphical program (most readers are 600(w)x800(h)). Add that image to Sigil. Create a new page and add the image to it (all options in the menu). Make sure that the new page is the first page in the document and most reader programs will pick it up as being the cover.
