Thanks for the tips but I still have a problem with the suggestions below. When I connect my KT, all my plugins on Calibre's toolbar disappear and only reappear when I disconnect the device. I created APNX files and manually copied them to the SDR folders and that worked.
But how do I accomplish the same task with the Kindle app for iOS? My converted .mobis do not display page numbers in the app. Calibre is set to the default of inserting the [PDOC] tag during conversion. Should I remove this or replace it with something else prior to conversion? How can page numbers be incorporated in the converted mobi file without creating a separate (APNX) file?
I am not familiar with the Kindle app for iOS (iPad) -- any iPad users here that know how to solve this?

To eliminate most of the variables, try the steps below:
1) connect your Touch & open Calibre
2) chose your APNX plugin -> "generate from file" -> selected the mobi/azw book on your kindle, not on your computer
3) when it asks you where to save the file, also choose the book's SDR folder on your kindle
4) unmount the Touch and reload the book.
If the above works for you, at least you'll know that the plugin works. So all you'll need to do is figure which steps you were doing went wrong before.
