problem can be accomplished by so many means that there is no better solution than what you are doing without knowing the code construct that is cauing this.
Conversion from PDF is a likely cause, which makes your
eyeball and fix technique one of the best.
Since this is a book likely in copyright, please DO NOT post more than a line or 2 of code if you need help with a REGEX replace.
