The debug trace shows that the Apple device driver is disabled in Calibre.
Note that if using the recommended method for Apple devices of using the Connect to iTunes method of interacting with Calibre this is the way you want it to be set. Maybe you had earlier followed the steps listed in the sticky and disabled the Calibre Apple device driver but forgotten you had done so?
