Part of the problem on a Kindle is that it has to use the FAT filesystem to allow easy USB export and FAT is not good at handling many files in one folder. Using Calibre, for example, you can build a folder hierarchy even on a Kindle and this seems to help.

Also, you may be experiencing a re-indexing bug that crops up sometimes. One fix is to disable indexing.
