Hopefully Kovid's advice has you up and running again.

As a general rule you should avoid PDF files for conversion and rarely should you ever use PDF as an intermediate format. General guidelines for converting Docx to mobi are to save the Docx as web page filtered. This creates a html file. Add the html to calibre then convert that file to mobi.
