Analogus, what you describe might be a result of generic file system corruption.
Could you test if deleting / adding files (multiple operations) breaks it in safe mode.

File name should also be in UTF8.
When was the last time you've used setup.exe to install PRS+? There was a bug in, mm, 2.0.14 I think, which requires full re-flash to fix.
