It depends on where you got the book....
For most non-DRM books I obtain, I can add the cover via calibre's conversion, then zap it on to my reader.

Places like Project gutenberg are plain vanilla epubs with no fancy covers, but I always add a cover that I like afterward.

With DRM stuff, I get what I get, and don't bother with that. I don't buy drm stuff, but I do borrow from the library or download the freebies.
