Can't be a cover in my library.
I deleted my entire library off of the server, leaving only the metadata.db.
The Server works, serves pages, etc, but uses the standard default cover image as f no cover images are available, and if you try to download any of the books you get an error when trying to download, but you can still search, view the book details, etc.
Server does the same thing, as soon as an iPhone (safari client) hits the site, it comes down with the same error (while the iPhone is being presented with the login prompt.) So, unless it is something in my metatdata.db that would be causing it...
which the only thing I can think of, that I have not tried yet, is on my laptop I have the kindle categories plugin installed, and a few books assigned to custom Kindle categories. This plugin is not installed on the linux server / nor my windows vps. This is the only thing I can think of that would have changed anything in the database that would not be present in a default database file.
