And if, like me, you send a book to your reader, decide it needs work to be more readable, make a copy, futz with it, merge it back in, etc. -- there's a reasonable chance it won't be recognized as being the same as the book on the reader.
It turns out the matching protocol is fairly complex, I quickly stopped thinking about it.
Instead I occasionally check my reader for 'orphans' --
go to device view and search/sort the In Library column (inlibrary:false),
and for 'doubles' --
in library view search/sort the On Device column (ondevice:"books"),
and get rid of them.
