Calibre is quite ahppy to store multiple records for the same book (the internal ID will be different). It will only be ignoring the duplicate bwhile adding the book ecause that is what you have set under Preferences->Adding Books.
You could change that setting and then get books added as new ones every time.
An alternative would be to add an empty book record via the drop-down on the Add Books menu icon; set up the metadata appropriately and then add the book format via the Edit Metadata dialog.
