When you added the .htm file to Calibre it should have resulted in a ZIP file which included the images as well as the .htm file. If this is not happening then the issue may be in the way the .htm is referencing the images.

The layout you mention is not the one that Word normally creates when saving in HTML format? If you use the option in word to save as "Web Page (filtered)" I would expect to see the .htm file accompanied by the images in a folder named like the .htm file.
