The reason for the bubbles under the film is dust that has settled on the screen in the short time between cleaning the screen and applying the protector.
On a PDA forum they advised to go to the bathroom, turn on the shower and wait until the room is steamed up to apply the screen protector. Apparently if the room is steamy there aren't any dust particles floating around.

To avoid bubbles I always put on the protector in a corner or at the edge and use a credit card to push the rest of the protector down from the edge/corner outward.
