Thanks to those of you who identified annotations as a possible culprit. I do a great deal of highlighting and annotating while I read, so I think the next step for me will be to export some of my annotations and see the results of that.

I really did not keep tabs of the SD cards I tried. One thing I noticed is that if you go above 16GB, there is a noticeable slowdown (I think on paper it's supposed to support 32gb...).
I now have a SanDisk SDHC2 16gb card in mine, and that performs very well.
Also, I have a SONY MemoryStick Pro Duo Mark2 8gb and that seems pretty fast as well.
Wish I could provide more detail on the cards that were slow... but I don't have them anymore. Only thing I remember, a 32gb SanDisk I had was noticeably slower.
Another option i can think of is to install PRS+, move content to the SD card and disable indexing. Not sure what the drawbacks are - maybe someone with more tech understanding can explain what the benefits of indexing are.... I have PRS+, but I have not disabled indexing, because, ever since I move content to the SD card, things have been much smoother than when the same content was sitting on the internal memory... As I have said before, that makes no sense to me, but others have noticed it as well.
