Did you try using Amazon's own KindleGen? It seems to do a much better job than Calibre on my hand-crafted EPUBs; I haven't tried many commercial EPUBs yet.

If you're on Windows, it's possible to set up a right-click -> Send to Kindle feature that points to a script that will call KindleGen and then copy the new file over.
