Well, the title consists only of 5 capitalized letters, the way the author intended it to be. It's like say
CIA, Black Ops. How would calibre do a proper title case?
Cia, Black Ops?
Calibre has probably a built-in safe acronyms list - CIA, FBI, NATO, UN...

If I change the title to
QBVII, Queens Bench Courtroom Number Seven, calibre accepts the capitalized
QBVII, but not if the title consists only of
QBVII.
CIA, Black Ops is also accepted, but a title like
CIA will be converted to
Cia.
