just switched on my 950, which I didn't touch for several days. PRS+ is there. (well, check for "usb connected" is very reliable)
Did you try rebooting / resetting the reader?
I have no idea what could cause effects that you describe. Maybe partition. where PRS+ is stored was damaged in a strange way? Hm. Ping me (kartu3 at gmail ) next time it happens, we could do some tests to figure what's going wrong.
