I'm not that concerned with freehand notes with ePubs. And since you point out the lack of infrastructure for such work in ePubs, it's a good thing I'm not

I only use freehand when doing 'red ink' markups for corrections, which are almost exclusively PDFs. Highlighting and text notes are sufficient for my other annotations.
However, when I need to draw a line on a graph, or write out a equation, or other things that are better conveyed with some sort of drawing, I use freehand, even if the accuracy isn't the best with the touchscreen.
The freehand isn't a deal breaker for me. The document is usually a throw-away, to be replaced by a corrected version. I don't need to use Mantano's organization options to manage those documents, and just sticking them in a folder works fine. It just would be nice to use one PDF app instead of two
