The Asus eeeNote Does everything you want except phone and I dont know if its been hacked enough to allow for running your own programs.

The Samsung Galaxy Note was just announced. Its Android so programming for it is possible and its a phone. The display is S-AMOLED which should be easier on the eyes than LCDs

I would also add the Asus Eee Pad Transformer to this list as well, with the keyboard dock basicaly turning it into a clamshell netbook, it has Polaris Office installed for your notes and organization, as far as making calls you can use Google Voice and Google Talk with it. and $399USD for the tablet and $150USD for the keyboard dock, it's the best deal going IMHO

Also If the screen size of the Samsung Galaxy Note isn't too small at 5inches, then that one actually should hit all the right notes on your wish list.

So I would look at
1. Samsung Galaxy Note fist and then the Asus Eee Pad Transformer.
