Go to Preferences - Toolbar click the dropdown arrow on the right and select the main toolbar when device is connected and add the send to device icon back to the toolbar.

I'm guessing you (or someone in your home) is familiar with this menu since someone using your instance of calibre went to this menu and removed the icon.
