As I said, the last part applies generally for me (if I don't open it for, say, more than two weeks).
For me, it's a case-by-case basis, but as you say there's a (soft) time limit there. If enough of the book sticks with me that I don't have to start again, I won't start over, but if I've lost the thread of thought, I'll start again. (If I decide to keep reading and not just toss it.)
