I prefer to have direct control over the end result, as proper formatting is very important to me. I hand-coded the HTML for the Kindle edition, and I'm very pleased with the results. But I don't want to exclude international ePub purchasers, so it may be necessary to include Smashwords as a distribution channel.

If you submit a really clean Word doc, SW generally does a good job. You could download a couple of my samples and judge whether they're up to your standards. I didn't do anything except be very careful with the formatting of my Word doc.
[I should say, they do a good epub. The .pdb--not so much.]
